REQUIREMENTS FOR OBTAINING/OR RENEWING AN EXPIRED LIBRARY CARDIn addition to completing an application form, all applicants must provide the following: NEW FAIRFIELD RESIDENTS: PHOTO ID and Proof of residency (i.e. Current driver's license, current mail delivery, tax bill, deed, etc). If patron is known to staff, a photo ID is not required for renewal.
CONNECTICUT RESIDENTS: Current (not expired) library card from home town library. Some library privileges may be limited.
OUT OF STATE RESIDENTS: PHOTO ID and Proof of current address with current driver's license or proof of current mail delivery. A yearly fee of $25.00 for the first individual, plus an additional $5.00 for each family member. CHILDREN: under 14 must have a parent or guardian signature. YOUNG ADULTS (14-18 YEARS): may get a library card without a parent/guardian signature if they meet the requirements of obtaining a card: PHOTO ID. The young adult (14-18) will be restricted from taking videos until a parent/guardian signs the form granting permission. |
